We respect your privacy and handle your personal and health information in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles. Information shared in sessions is kept confidential and will only be disclosed with your written consent, unless required by law (e.g., risk of harm, child protection, or a court order). By signing our consent form, you consent to the collection, storage, and use of your personal information for the purpose of providing psychological services.

Collection of Personal Information
We collect the required personal information to deliver our psychological services effectively and maintain accurate records. This may include:

  • Your name, contact details, and demographic information.

  • Medical history, psychological assessments, and treatment plans.

  • Billing and payment details.

  • Medicare number, details of your referring doctor or other related referral.

  • We collect clinical information through in-person appointments, Telehealth appointments and phone consults.

  • Personal information is stored through a trusted practice management software (Zanda).

Use of Personal Information
Your personal information is used for the following purposes:

  • The delivery of psychological services, including intervention and assessments.

  • Booking appointments.

  • For internal record keeping.

  • Managing billing and processing payments.

  • For report writing if required and consent is given.

  • Keeping accurate records in line with legal and professional requirements.

Disclosure of Personal Information
We may disclose your personal information in the following circumstances:

  • With your provided consent.

  • To other healthcare professionals involved in your care. This includes corresponding with your referring GP, as per Medicare guidelines. This may also include collaborating with Work Cover, DVA, or third party insurances when this is required.

  • With consent, personal information may be written within assessment reports or NDIS review letters. These reports will not be sent to other professionals/individuals without prior consent from you.

  • When required by law or court order.

  • In cases where your safety or the safety of others is at risk.

  • At Clear Wave Psychology we at times engage in clinical supervision to continue to ensure the level of support we provide is of a high quality. Your personal information will remain confidential.

  • We store personal information through our practice management software (Zanda), Nova-psych, and Stripe.

  • We keep your personal information only as long as needed for its purpose or by law. After that, it is securely deleted. Health records are kept for at least 7 years after your last health service. For children, records are kept until at least age 25. When we destroy a record, we note your name, the destruction date, and the record’s time period.

Your rights:

You are entitled to access the personal information we hold about you and request updates if needed. If you believe any of the information is inaccurate, incomplete, or no longer current, please reach out to us to have it corrected.
