Welcome to Clear Wave Psychology. This Service Agreement outlines important information regarding your psychological care, including consent, confidentiality, fees, cancellations, and payment terms. Please read this document carefully before proceeding with services.
1. Consent to Psychological Services
By engaging in psychological services with Clear Wave Psychology, you acknowledge and consent to:
The collection, use, and secure storage of your personal and health information for the purpose of providing psychological services.
The provision of assessment and/or treatment by a registered psychologist in accordance with professional and ethical standards.
The exchange of information with relevant third parties only with your written consent, or when legally required (e.g., risk of harm, child protection concerns, or under court order).
2. Privacy & Confidentiality
Clear Wave Psychology is committed to protecting your privacy. All personal and health information is managed in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles. (Please see privacy policy for more information).
Information shared during sessions is confidential and will not be disclosed without your consent, except in the following circumstances:
Where there is a risk of serious harm to yourself or others
Where there are concerns related to child protection
When disclosure is required by law (e.g., a court subpoena)
3. Service Fees
Clear Wave Psychology offers the following services and standard rates:
Standard sessions (General Psychologist): $225 per 50-minute session
(Medicare rebate of $98.95 available with a valid Mental Health Care Plan)NDIS-funded sessions: Standard rate per session
Work Cover and DVA: Standard rate per session
Payment is due on the day of your appointment.
Please refer to the cancellation policy below for information on fees for missed or cancelled appointments.
4. Credit Card Authority & Payment Processing
Clear Wave Psychology uses Stripe, a secure online platform, to process all card payments.
By providing your credit/debit card details, you consent to the following:
Fees for services (including late cancellation or non-attendance fees) may be charged to your card after the appointment.
Stripe may retry unsuccessful payments and contact you to arrange alternative payment methods if needed.
All payment details are stored securely and managed in accordance with industry standards.
You confirm that you are the authorised cardholder.
5. Cancellation Policy
We understand that unforeseen circumstances can arise. To ensure fair access to appointments for all clients, we require:
Minimum 3 business days’ notice for rescheduling or cancelling an appointment.
Cancellations made within 2 business days of your appointment will incur a 50% session fee.
Cancellations with less than 24 hours’ notice (1 business day), or non-attendance, will be charged the full session fee.
Exceptions may be granted in the case of genuine emergencies, at the discretion of your practitioner.
Please note: If you arrive late, your session time may be reduced, but the full session fee still applies.
Payment for late cancellations or missed appointments is required before booking your next session.
6. Agreement Acknowledgement
By signing or electronically agreeing to this Service Agreement, you confirm that you:
Have read and understood the terms outlined above
Consent to psychological services under these conditions
Acknowledge responsibility for payment and any applicable fees
Understand your rights to privacy and confidentiality
